Hyper Radiesse vs. Filler: Which One Actually Helps Crepey Skin?

By Maddie, Master Esthetician and Owner, Bocaba Medical Spa. Medically reviewed by Mistie Sue Manning, NP, Medical Director.

If your skin looks thin and crinkly, like tissue paper, regular filler usually isn't the answer. Hyper Radiesse is. Filler adds volume to one spot. Hyper Radiesse spreads thin across an area and signals your skin to build its own collagen. Crepey skin is a quality problem, not a volume problem, so you want the product that improves quality.

What "crepey" actually means

Crepey skin happens when collagen and elastin (the fibers that keep skin thick and springy) break down. Sun, time, weight changes, and genetics all play a part. You see it most on the neck, chest, inner arms, above the knees, and the backs of the hands. The skin isn't sagging so much as getting thinner.

Why regular filler isn't built for this

Hyaluronic acid (HA) filler, like Juvederm or Restylane, is great at what it does. It adds volume where volume has been lost: lips, cheeks, under-eye hollows, a jawline. It works like filling in a dip.

Crepey skin doesn't have a dip to fill. If you spread HA filler across a large thin area, it can look puffy or uneven, and it doesn't change the skin itself.

How Hyper Radiesse works

Radiesse is made of calcium hydroxylapatite (CaHA), tiny particles that kick your body into making new collagen and elastin. "Hyper" means we dilute it with saline so it spreads thinly and evenly under a wide area of skin. There's no added bulk. The particles work as a collagen signal, and over the next few months your skin gets thicker, smoother, and firmer.

Think of filler as adding padding, and Hyper Radiesse as thickening the fabric.

Side by side

  • HA filler: Adds volume to a specific spot. Best for lips, cheeks, under-eyes, and jawline. Results right away. Little to no downtime.

  • Hyper Radiesse: Rebuilds collagen across an area. Best for a crepey neck, chest, arms, knees, and hands. Results build gradually over about 3 months. Little to no downtime.

What to expect at your appointment

Mistie maps the area, then places the diluted product with a thin cannula, a blunt-tip tool that's gentler than a needle. Most appointments take under an hour. You may have a little swelling or bruising for a few days, and you can go back to your day right after.

Most people do 2 sessions, spaced a couple of months apart, for the best result. Hyper Radiesse is $1,275 per session at Bocaba.

Who's a good candidate

You're likely a good fit if your skin looks thin, crinkly, or loose in texture, and you want natural improvement rather than a dramatic change. If there's a lot of loose, hanging skin, we'll talk honestly about what injectables can and can't do.

Using diluted Radiesse on the neck, chest, and body is an off-label use, which is common in aesthetic medicine. Mistie will walk you through it at your consult.

FAQ

How long does Hyper Radiesse last?

The collagen you build lasts well beyond the product itself. Most clients see results for 12–18 months, and many do a touch-up once a year.

Can I combine it with other treatments?

Yes. Hyper Radiesse pairs well with RF microneedling or CO2 laser, which improve skin from the surface while Radiesse works underneath.

Is it the same as Sculptra?

Both build collagen, but they use different ingredients. We're happy to talk through both at your consultation.
