How Many RF Microneedling Sessions Does It Take for Acne Scars?
By Maddie, Master Esthetician and Owner, Bocaba Medical Spa
Most people with acne scarring need 3 to 4 RF microneedling sessions, spaced about 4–6 weeks apart. Deeper or more widespread scarring can take more, and sometimes the best results come from pairing RF with CO2 laser. The exact number depends on what kind of scars you have, how deep they are, and how your skin builds collagen.
I'll be upfront about one thing: no treatment erases acne scars completely. What a good plan does is soften them enough that you stop noticing them first, and stop covering them up.
Why it takes more than one session
Acne scars are areas where collagen got damaged or lost when a breakout healed. RF microneedling works by heating the deeper layer of skin so your body builds new collagen there. Each session builds a layer. It's a lot like strength training: one workout doesn't change much, but a consistent plan does.
Collagen also takes about three months to fully form. So the results from session one are still showing up while you're in for session three.
Which scars respond best
Not all acne scars are the same shape, and the shape changes the plan.
Rolling scars (soft, wavy dips that make skin look uneven): respond very well.
Boxcar scars (wider dents with sharp edges): respond well, often better with CO2.
Ice pick scars (narrow, deep pinpoint holes): the hardest to treat with any device.
Most people have a mix, which is why I look at your skin in person before I give you a number.
When we add CO2 laser
RF microneedling rebuilds collagen underneath. CO2 laser resurfaces the top layer. For scarring that affects both, using both gets you further than doing either one alone.
That's the idea behind our Ultimate Duo, which pairs CoolPeel and RF microneedling in one visit for $1,400. Expect about 5–7 days of downtime with the Duo, compared with 1–2 days for RF alone.
What a typical plan looks like
Consultation. We look at your scar types, skin tone, and whether you still have active breakouts.
Get breakouts under control first. Treating scars while you're still breaking out is like painting a wall that's still getting dented. We'll set you up with a home routine to calm things down.
Series of 3 RF sessions, about 4–6 weeks apart.
Reassess at 3 months. If deeper scars remain, we add a session or a CO2 treatment.
What results look like
Most clients notice smoother texture after the first or second session. The bigger change shows up 3–6 months after the series, once all that new collagen settles in. Makeup goes on smoother, light hits your skin more evenly, and the scars look shallower.
FAQ
Does RF microneedling work on old acne scars?
Yes. Scars from 10 or 20 years ago can still improve, because the treatment rebuilds collagen no matter how old the scar is.
Is it safe for darker skin tones?
RF microneedling is one of the safer options for deeper skin tones, because the heat is delivered below the surface. We still adjust settings for your skin.
How much does it cost?
At Bocaba, RF microneedling is $699 per session or $1,799 for a series of three.
